Instructions

1. Please fill in the fields below and print this form. Enclose the printed form with your application. Please note that only students who have a cumulative GPA of 3.3 will be considered. Do not apply if you do not have a 3.3 GPA.
2. Also include a one paragraph summary regarding your achievement, and a letter of recommendation that addresses the achievement. The achievement should be an exceptional accomplishment beyond standard curriculum and course requirements. For example, exceptional community service or an exceptionally innovative project in a class. The letter should be printed on letterhead, signed, include name, title and contact information, and must be included with your application in a sealed envelope.
3. Submit the form, letter, and one paragraph summary by the application deadline to:
